Course Overview
Effective incident reporting begins with recognising what has occurred and responding to immediate safety and wellbeing needs. Once the situation is stable, workers must create an accurate record that explains what happened, what actions were taken and what follow-up is still required.
This course helps support workers distinguish incidents, near misses, shift notes, ABC records and incident reports. It develops practical skills in writing clear timelines, separating observations from assumptions, recording participant statements, documenting injuries and notifications, escalating serious concerns and correcting records appropriately.
Workers will also explore how complete, objective documentation can reveal recurring risks and support safer, more consistent practice across the service.
